elle dit que je devrais lui acheter sa vieille voiture.

As far as I understand, here we have 2 options:

1) she says I should buy her her old car.

2) she says I should buy her old car from her.

and they have different meanings, which can only be understood from context, right?

There are actually 8 options, because lui can also mean "for/from him" and sa voiture can also mean "his car."

So in addition to your two, there's also

1) She says I should buy him her old car ... buy her his old car ... buy him his old car.

2) She says I should buy her old car from him ... his old car from her ... his old car from him.

To cut out a few possibilities if the context doesn't make it crystal clear, you'd be well advised to use pour + lui or elle in the (1) scenarios and à + lui or elle in the (2) scenarios.

Thanks for your reply.
I guessed it was, but I wasn't sure.
Confirming the ambiguity of lui helped a lot.

If I understood correctly, the original sentence could be replaced with the following:

elle dit que je devrais acheter pour elle sa vieille voiture.

elle dit que je devrais acheter à elle sa vieille voiture.

P.S. I don't know if I put (pour elle / à elle) in the right place.

You can put pour / à elle either after acheter, as you did, or at the end of the sentence.
